Indemnity and Liability Waiver

By enrolling my child in the HOPfit Kids Holiday Club programme, I, the undersigned parent/legal guardian, acknowledge and agree to the following:

1. I understand that the holiday club involves physical activity and group participation, which carry inherent risks. While all reasonable precautions will be taken to ensure the safety and well-being of all participants, I acknowledge that minor injuries such as bumps, bruises, or scrapes may occur in the normal course of physical activity.

2. I hereby release, indemnify, and hold harmless HOPfit (Pty) Ltd, their staff, volunteers, affiliates, and the host venue (Generation School Sandown Institute), from any and all liability for injury, loss, damage, or claims of any kind that may arise during or in connection with my child’s participation in the holiday club, except in the case of proven negligence or wilful misconduct.

3. I confirm that I have disclosed all relevant medical, behavioural, and emergency information that may impact my child’s participation in the programme. I acknowledge that failure to do so may affect the ability of staff to care for my child appropriately and safely.

4. In the event of a medical emergency, I authorise the holiday club staff to seek appropriate medical care for my child and agree that any medical expenses incurred will be my responsibility.

By checking the checkbox and submitting the registration form, I confirm that I have read, understood, and agree to the above indemnity and waiver terms.
